Very Early Scan due to ectopic scare... question

Here are my betas so far:
9dp3dt (12 DPO) 41
11dp3dt (14 DPO) 112
13dp3dt (16 DPO) 378
17dp3dt (20 DPO) 3769 hcg and progesterone 77

I was 4w6d pg at this reading and I went in for an U/S due to a small watery bleed 3 days prior. They saw NOTHING on U/S- nothing at all expect a burst cyst and my ovaries looked "complex" whatever that means. RE was very concerned about ectopic at this point.

2 days later on U/S the tech saw just one sac measuring 4w6d (I was 5w1d, so this is two days behind). HCG was 6608 (at 22 DPO). He is no longer concerned about ectopic but this beta seems way too high to just see a sac with no embryo or fetal pole yet. I am hoping it is twins and the other is just not visible yet- but this is why my numbers are so high.

My betas were initially doubling every 29 hours and this has now slowed to 59 hours. I was so hopeful for twins with my early numbers. Now I am worried about even one sticking! We transferred two 3-day old embies. One was 8-cell grade B+ and one was 6-cell grade A-.

Has anyone had a scan very early (5w1d) and only seen one sac, but gone onto have twins?

There are lots of people who haven't seen the second sack until around 7-8 weeks, but by then most will see it.

At 5 weeks your ultrasound wouldn't show much, but by 6-7 weeks you should be able to detect a heartbeat. It's hard but just hang in there and you'll know soon enough.

Holleigh, My situation wasn't/isn't exactly like yours but perhaps it will help. I didn't have as many beta's as you, or quite as early of a scan, but here was my situation:

I never knew my exact 1st beta, which was 9dp5dt, but I believe it was around 115. I had my 2nd beta at 11dp5dt and it was 418. At my 1st scan, I was 6 wks, 2 days and we had one gestational sac/yolk sac with fetal pole and heartbeat; there was a 2nd gestational sac with yolk sac, but nothing else visible. At the time RE thought perhaps it was Vanishing Twin, and just never continued development and we would have a singleton pregnancy. I returned for a repeat scan at 8 wks, 2 days. At that point, both were clearly visible, and Baby B had progressed to seeing a heartbeat, etc. The concern then was that Baby B was a full week behind Baby A, so it still could be early signs of Vanishing Twin. I had a repeat scan at 10 wks and both were still progressing with very strong heart rates (177 and 178), yet B continues to be 1 wk, 1 day behind.

I am now 12 wks, 3 days and waiting until 6/18 for my next scan. We have no idea if there will still be two, due to the large gap in development rate between the two there is a 50/50 chance the 2nd is slowly on the decline. I guess how this could be like yours is, if similar to me you could have twins and the 2nd is a week or more behind in development than the other, which is why at a 5 wk scan you wouldn't see the 2nd yet. remember, mine was a whole week later than yours and all we saw was a sac...a lot happens in just a day or two.

Thank you Sam & Paris. I am just praying that it was too early. In the two days between scan #1 and scan#2 the gestational sac appeared. So hopefully a day or two later the second one appeared!

My next U/S is a week from today. I should be 6w3d so I have to keep positive that we will see a beautiful beating heart or two!

No symptoms at all of ectopic. RE is supposed to call me today with the doctors report of my second scan since the tech told me something different than she wrote in the report.

This 2ww is worse than the first one by far.

And Paris... just want to wish you all the best at your next scan. I think it is amazing and wonderful that both babies have such strong heartbeats. I believe they just implanted at different times. Thanks again... XO~Holleigh
