Success w/ ICSI after biosy (TESE?)
I had open biopsy in Nov., in which my RE/urologist confirmed blockage due to scar tissue from a childhood surgery. He also said there was some production problem but he was able to find some sperms which were sent to an IVF center for freeze.
Recently, we consulted with an IVF specialist at the center. The embryologist in his notes mentioned one out of 10 sperms motile seen. Although we have 2 vials frozen, the IVF specialist put our chances to 10% (sperms may not survive thaw or may not be able to fertilize). My RE/urologist didn't say anything negative about the quality of sperms; I'm confused whether we should go ahead with what's frozen or do another TESE. Please let us know if any one else has experienced similar issues (fertilization) with sperms extracted through biopsy. What would you recommend we should do to improve our chances: should we get a fresh TESE or mTESE done? Thanks,

Hey scar,
We're going through something similar. I had no sperm in my ejaculate, but a biopsy (which confirmed my condition was non-obstructive and was done in June) yielded a healthy number of sperm for us. In our case, we were supposed to try the ivf during the December cycle, but my urologist opted to wait until March cycle in order to give me ample time to heal. Also, he'll be on call the day they thaw my samples in case there's no luck in order to perform an aspiration. If you were given a 10% chance, I would ask what backup plans, if any, are they putting in place in order to give you a better chance. Although, they're able to do this in conjunction because we're having everything done at the same hospital. Keep us up to speed with the process...I'll be doing the same. Hi there. So glad I found this thread. My husband and i are going through the same thing. We start our IVF this May and that is when they will do his TESE. We won't know until the day before they retrieve my eggs if he has anything. I am just so worried.
I have some questions that maybe I can get some reasurrance that there is hope for us. My husband in 33 and he has an FSH level of 23, has large samples, high fructose. His urologist is just sort of puzzled that they couldn't find anything in 4 samples we did since October 2008. I am worried about his FSH level being a 23 as they said he should be between 1-18. What were your FSH results and lab work. I just need some hope for us. What is the difference between obstructive and non-obstcutive azoospermia? Our urologist said that it sounds like a blockage and that the tubes weren't developed at birth for the sperm. He is giving us a 50/60 percent chance. We too are going through the same thing, kinda. My husband had a vasectomy. A reversal, and still nothing. He is having MESA done on Thursday. I am surprised your starting a cycle before aspiration is done. My doctor won't start me until sperm is aspirated since there is no guarantee and everything is so expensive. By the way, I have undergone all pre-testing and there are no problems on my side. My Dr gave us the same, around 50 percent chance of success per ICSI. Glad to find others who are going through the same procedures.

KKNY~ Our Dr's want us to have a donor back up if they dont' find anything. My husband and i have never had kids as we just got married in 2007. So, if they don't find anything, we are freezing my eggs to decide what we want to do from there. It's difficult for my husband, trying to soak in that he may never have a child of his sperm. So, right now, our Dr's said move forward with the TESE then my egg retrieval. We may ask a brother for a donor, but we feel that we don't want to get into that until we know for sure. Our Dr's tell us it's a 9 month recovery after the male surgery and it would put us back even more. I am 36 years old this year and so they recommend to just go ahead with everything. I started an IVF cycle in Jan, but they canceled it. I wasn't repsonding to the formula they gave me. I am doing some DHEA therapy and acupunture and go back in May to start another cycle. Urologists like obstructive better since it's a matter of removing or circumventing the obstruction. with unobstructive, it's usually a congenital problem or extremely low sperm production for any number of reasons. I'm no urologist, but hope that helps somewhat. My frozen samples have about a 30% chance of survining the thaw, which is why my urologist will be on call the day of thawing and egg retrieval (my wife's perfectly fine) in case they don't survive and a tese is necessary. |
