question on SIRM intralipids

Hi all, I am trying to understand the dosage for intralipids. It seems like SIRM infuses the entire bag (100ml of 20% intralipid), but then I read that Dr. Coulam only infuses 4ml? That is quite a big difference. Is there any reaction to getting that much intralipid?

Hi,

I had a consult with dr.t of sirm and recently did the 100ml of intralipids infused and I had absolutely no reactions. I had more reaction from ivig that I have done in the past.
I also had a consult with dr. Coulam and I believe she has a lot less experrience with intralipids. Dr. T told me that they are actually replacing all of their ivig protocols with intralipids because of its success..
My concern with intralipids is that 100ml = 200kcal= 200000 calories= fat @ss. Lol. If u find any ifo different please keep me posted.

lw962, I didn't realize it was that many calories! Did you gain weight afterwards? wow, I'll have to do some more checking on that.

actually since doing the intralipids ive given up eating after 6pm with my last meal usually being 3pm and i have lost a pound. but please if you find any contradictory info let me know.

When did you have the intralipid -was it 7 days prior to ET? Do they do a couple of infusions prior to ET or just the one?

I checked into the calories in the intralipid. I understood it to be 2000 Calories for 100ml. At least that is how I understood all the sites I have been reading. I'm going to keep checking.

babee- when i ordered my intralipid from ed of columbia pharamcy, the bag had some explanation including ingredients and calories/100ml. did you order your intralipids from there as well? im out of state and they mailed mine to my home and as i read it, its says that each 100ml = 200kcal. it wasnt even writed in calories but rather kcal. look on the intralipid bag to confirm. thanks for helping out with the research b/c i couldnt find anything to confirm. maybe i should call the pharmacy?

btw, dh doesnt want me doing anymore ivf's and im actually relieved that he made me stop b/c after 3 failers and a chemical im exhausted. but we went back to doing medicated iui's. and the intralipids were given cd7, which should of been 7d prior to iui. im currently 3dpiui.

Can anyone help me out? I am trying to locate a doctor that will enter into a collaborative arrangement with my current doctors at Shady Grove. The next step for me would be a donor egg cycle at Shady Grove in conjunction with Intralipid Therapy. I have elevated NK cells and the recommendation would be Intralipid Therapy...I will not do IVIG. The doctor making the recommendation ordered the testing and discussed the results with me. However, he will not work with my current doctors even though I contacted him from a referral at Shady Grove. After 3 years at Shady Grove, I do not want to start over again at another facility. Can someone tell me the easiest way to get Intralipids and how that process works? Thanks!

You may be able to go to Dr. Coulam at Rhinehart center in IL for the intralipids. I ran into the same problem. SIRM wouldn't treat me unless I became their patient and travelilng back and forth wasn't feasiable. Fortunately, I found a doctor in my area who will do my intralipids but again unless you are their patient, I don't think they will do it. I actually changed RE's but my former RE couldn't do anymore for me and wasn't on the same page re: nk cells. Dr. Coulam will work with your current doctor. She moderates an immune bb at incidd so you may want to check it out and contact her. Good luck!

i called Dr. Tortoriello of SIRM. i had a free telephone consult with him after faxing all my immune test results. he e-mailed his recommendations, including 100ml intralipids, which i forwarded to my local doc. my local clinic uses 4ml of intralipids so it was a huge change for my clinic but my doc was willing to make the Rx. my local doc wrote the rx for intralipids and we followed Dr.T recommendations to a tee.
Dr. Tortoriello is kind and is willing to work with your doc. he is so nice just tell him what you are looking for and he will do his best to comply. btw, dr.t no longer uses ivig at his clinic bc of the effectiveness/successs and inexpensiveness. they recommended columbia pharmacy, they were great!
