Hey Cristy!
My DH had this procedure twice, one was just diagnostic (see if there was anything there before we cycled) and was done under IV sedation, the sedation threw him for more of a loop than the pain from the procedure. It is basically just a flexible needle/tube that they insert and blindly aspirate tissue and examine the tissue later. The one that was done right before my ER was done only under local (lidocaine) and he was in no discomfort. Unfortunately the diagnostic had some sperm but the actual one did not so we had to abandon the cycle before ER (because we didnt have a donor sperm back up) Now we have a new urologist who will be doing a testicular microdissection (under general anesthesia, open dissection not just a needle, and they will examine the tissue/tubules under a microscope before removing them) so I am guessing he going to be in more pain - that and they gave him a huge rx for percocet which isnt a good sign...lol
I will keep you updated, hoping your TESA goes well! fingers crossed

Cristy, I am so sorry, unfortunately there is nothing you can say to him to make him feel any better just be there for him. We had always suspected the no sperm results given his history of chemo but when you actually get confirmation its still a big blow.
I dont know what your next step is but I can say that we have a new urologist in MD (that we travel 3 hrs to see) and she started my DH on Arimadex (which is actually a breast cancer med, it blocks testosterone from turning into estrogen) and his testosteron is up from 127 to 380 so she has more hope for our next procedure in Sept. You can look it up, I dont know if its an option for you, its a testicular microdissection.....our urologist website is Maryland | Virginia | Washington, D.C | labiaplasty |  male infertility | vasectomy reversal | labia minora reduction | labia majora remodeling| vaginoplasty | vaginal rejuvenation | vaginal beautification | male infertility | vasovasostomy | ge.
Now going to a bigger fertility center we are using a donor back up for the next cycle if this procedure finds zero again and their protocol have anyone with the possibility of using donor anything see a therapist and at first I was skeptical but omg did it help both my DH and myself.
I hope everything works out for you no matter your next step. If I can help with anything (info etc...) let me knowMe: 30 y/o, DH: 30 y/o (history of chemotherapy)

Sharing our story as its similar to yours. As if our poor responding eggs weren't enough, his dx of no sperm has been tough.
He had a procedure done for unblocking, but surgery was a failure. Thankfully when they operated they took tissue out and froze it in 2009. He had a tesa trial run two weeks go and they found swimmers and they froze them. By the time of the Er this week, his tesa resulted in 0 swimmers. So thy unfroze the sample from two weeks ago but the freezing damaged them. Luckily they went to the 2009 frozen batch with tissue and they found swimmers which fertilized our current 4 embies that are in the Von for the 2ww. Longgggg journey. His tesas have been local anesthesia with locadaine but it's this injection that has hurt him a lot and not the actual procedure.
