In the event of illness or minor injury P. A. in Cville should be contacted. They see Dr T there. If it’s a serious illness or injury they are to be taken to MJH or to the U of V Hospital in Cville---in that order.

I think it needs to be witnessed.
JMHO ... but I don't think you should specify doctors or hospitals ... what if something serious came up and they wanted to airlift your child to a certain place for some limitted-availability treatment --- your letter denies that by limitting the hospitals they can go to. If you trust your children's aunt and grandparent to care for them, then give them the authority to make decisions in your absence.
That said, you might want to say "their regular pediatrician is _______" . That way authorities know where to look if medical records are needed ... and if there's anything like alergies that may need to be known about immediately, list those.Steph
