Update...I am out for 3 cycles...

Hi Friends!

Well, I had my first appointment with the RE and he said that I have insulin intolerence and put me on Metformin 1000mg twice a day. He wants me to loose weight and said Metformin shows its effect after about 2 months. He doesn't want me to take any clomid or anything.

He also didn't want me to start with injectables right away. He says the risks with injectables are too great to jump right into them. He wants me to try everything else, before I start with injectables. Of course I got that multiples speech and the loose weight speech.. arggh!!

OK. I realize that I am over weight but its not like I am real real big girl. I am 4'11" and 199 lbs.

I don't know.. I am a little discouraged but agreed to give this a try. I pray to God that he knows what he is doing, other wise I will be waisting precious 3 months with this...

During this time, I will be checking on you guys every now and than. I hope you all get your BFP's soon. My thoughts and prayers are with you all.

Please wish me good luck with this plan too. I hope it works!

Take care y'all!

Hmmm... is the the only RE in your area? I am overweight and my RE never even BROUGHT UP weight. Heck, he never even weighed me!

Crystal, my insurance only allows referral to two clinics. I guess, I could ask for a transfer to the other clinic, but I actually heard so much better things about this clinic (and not so good things on the other option)... I am afraid that it actualy could be worse if I go to this other clinic.. I don't know..

Here is what he told me: Since my husband has zero sperm, he says that I may not even have an infertility issue and that it may be as simple as timing error for me. He explained my low progesterone problem and my cycle lenght being only 25 days with me being over weight and having insulin resistance. He says because of me being over weight and the insulin issues, that my brain doesn't get the message that I need more progesterone. So he thinks, if I can fix the weight problem and the insulin problem with Metformin, I would have longer periods and ovulate on my own and therefore my only problem would be just a good timing with IUI.. I mean that is his theory.

I am so frustrated and confused... I don't know what I need to do.. Shall just trust this RE's instincts and go along with it (which is what I am doing currently) or should I see a way out and go to another RE? I wish I knew what is the RIGHT thing to do...

Horizon! I just saw your post and I'm sorry your RE wants you to wait for 3 cycles. I'm curious how did the RE know you have insulin intolerence? Did he run some tests on you? I'm wondering if you can get a second oppinion or something? I know how hard it is to wait - it's not like I'm getting any younger! (I'm about your age, 33)

I don't know what your RE said about multiples that scared you. I wish I'd have twins and get IF behind me for good! Why is that scary? I know it's harder on the mom's body, as well as the babies, but so many twins were delivered each year and vast majority of them are fine!!!

I dont' know about the timing error. If you had trigger shot, could the timing still be wrong?!

Regardless, you get my wishes! I wish you luck in whatever you decide!

Susan...

Gads Horizon! I'm sorry for the delay. Been there done that. Yuck!

Isn't it hard to trust when you have been given conflicting advice? Let us know what you decide. I am regreting not getting TWO opinions before I started with the clinic I am at.

Not sure what to do here either. Hugs!

Oh, to add one more thing...

I'm starting injectables in March. My RE did tell me injectables have much greater chance of multiples. I said, we'd be happy with twins. He said, he's worried about triplets. Well... I don't know what to say, but if I'm pregnant with triplets, I'm happy too! I'm more worried about not getting pregnant than having multiples at this point!!!

Susan & Dana - Thanks so much for your post.

Well, I don't want to bring anybody down or anything like that but the RE said that with triplets there is a great risk of unhealthy babies (mentally retarded and such), premature birth, still birth, etc in addition to the tremendous risk for me becoming diabetic and high blood pressure issues.
I wouldn't mind twins myself either, but don't want to have triplets if it would endanger their lives and well being. Besided I am a short gal, I really don't think I have enough room to accomodate 3 babies, lol

Anyway, he just spoke of so many negative stats regarding multiples, I was just taken back by it... If he wanted me to be scared of the injectables, he sure did his job well!!

As far as insulin intolerance goes, he actually didn't run any tests yet, but I have been checking my blood sugar level every year since I have diabetes in my mother side of the family. Fortunately, I am not diabetic but my fasting glucose levels have been more like around 90's and that is what he based on his conclusion I think. And I actually had a very recent blood work and fasting blood sugar test as a part of my annual check up. I haven't received my results yet but he said that he will request that info to review.

I will definetely be looking into the second opinion issue on monday. I hope my insurance doesn't create problem in getting the referral for the second opinion.

From my point of view, my main problem is the fact that I have lutheal phase defect. My cycles are only 25 days long and that doesn't give enough time for implantation. I have to work on getting my cycles to more like 28 or 30 long first before I start with injectables. I think if I use injectables and have 3-4 mature follicles, eventhough they may get fertilized, they are not going to be able to hang around due to LPD.

So I searched in internet and it looks like I need to take B6 vitamin for about 2 month to see the result. Some websites also suggests using naturel progesterone cream but the RE didn't want me to take anything like that.
I have added a strong vitamin B complex to my daily prenatal vitamin.

I guess, waiting for 2 month may not be that bad after all, IF vitamin B6 and the weight loss & metformin regimen is enough to lengthen my cycle to a minimum of 28 days.

My problem is that I DON'T KNOW if this plan will work and am afraid waisting time by not doing the right thing that will help me solve my LPD problem.

Once I have a longer cycle, I think my low progesterone problem is an easier problem to attack. There are prog. supps or creams, etc... I am not as concerned about the progesterone issue than I am with the short cycle lenght.

Anyway, sorry for the long post... I hope I can get a 2nd opinion even if they tell me the same thing. At leas that I would be more comfortable with the decision that I am making...

Anna - Thanks for your kind message.
Right now it feels like I am in an extended 2ww mode... Time seems to slow down and I just can't get there fast enough

Waiting is the hardest part of all... I am trying really hard to keep myself busy with other things.

I left a message at my OBGY regarding my wish for a 2nd opinion referral, but haven't heard back yet.

Sorry you have to wait some more, but detoxing etc is the best thing you can do for yourself and the upcoming cycle Hang in there.

You can read this to get a better idea. http://www.ivf1.com/effects-of-obesity/
