I think it all depends on your clinic? I'm not sure. Alot of clinics won't do blasts cuz they don't have the freezing techniques for 5 days or vice versa. I was always told that just because an embryo does not survive to 5 days in a petri dish does not mean it wouldnt have survived in a uterus. I'm sure you will get plenty of opinions here about this topic.

5 years ago and 1 year ago one of the RE's at the clinic in CA reiterated that for their clinic there is no difference in pregnancy rates whether you use 3 day embryos or 5 day embryos. We have used 3 day embryos for ALL of the transfers we have done. I think the uterus is the best place for the embryo and when it stays in the dish longer I think it is harder to have culture mediums that replicate the womb's fluid. Just my opinion. I don't have any research.

Use of blasts does not increase your chances of pg, just decreases the chances of multiples. This is because many RE's will transfer 3 3-day embies but only 2 5-day embies.
It really does depend on your clinic's procedures. Some embryologists do well growing embies out to 5 or 6 days. I believe it does require several different mediums though, so some clinics do better than others with managing this mix. My pg were all achieved by using 3 day embies. My RE's lab was NOT good at taking embies to blasts and my embies were 'only' average anyway and would not have had as good a chance at going to blast anyway. With my embies, my clinic watched them each day and told me to be ready starting day three to come in for transfer. They let them go until it looked like they might not continue going great guns and did the transfer then. So some of your clinic's choices might well be based on the quality of your embies. Some clinics have GREAT successes with taking embies to blasts. As I said before, if you end up with blasts, most RE's will put in fewer, so your chances of multiples is reduced (this is a GOOD thing). Your best bet is to ask your clinic what their success rates are with 3 and 5 day embies and see what their procedure is. Diana

My RE also used ICSI. He felt it improved the odds of fertilization.

My clinic does AH and ICSI with all embryos.
My thoughts on 3 vs. 5 day: I feel that if an embryo is going to survive long enough to become a full term live baby, it will survive for 5 days in a petri dish. We did a 3 day fresh transfer first which was negative. They let the rest of my embies go to 5 day and only froze 5 of them. I think at 5 days, you really know better if your embryo is a survivor and your RE can pick the best quality to transfer.

Similar to my ICSI reply. Our RE used the numbers of developing embryos guide what happened. With 32 fertized eggs they froze 10 day-2 and let the rest go to blast because they were developing well. We transfered 2 day 5 blasts and they let the rest go to day 6 and had 3 to freeze.
